Welcome to Park Farm Surgery

Currently we are no longer taking new patients so that health and safety of the existing patients on our list is not compromised.

How to register with our Doctors

Whilst we will be happy to register new patients to the practice providing you are living within the practice catchment area, we do not as a rule accept patients from other practices in the area unless those patients wishing to move practice can provide adequate reasons why they wish to move to this practice.

New patients will be registered with the practice and not an individual doctor. When you join the practice we ask you to fill in a registration form and invite you to attend a registration medical, which includes filling in a form about your past and current medical history, a short examination, including height, weight and blood pressure. Please bring along a urine specimen with you. This service is also extended to patients not seen in the surgery for three years.

This approach applies to any patients, visitors and any persons working within the practice demonstrating any of the above behaviour towards patients, visitors or staff.

The doctors are committed to do everything possible to protect staff, patients and visitors from unacceptable behaviour and to support zero tolerance to any incident that causes hurt, harm damage or stress.

This policy applies throughout the premises of Park Farm Surgery, including the car park and surrounding grounds. It also applies to any doctor or employee of the practice whilst on duties away from the surgery if their duties are related to the business of the practice.
